What this code covers

Source · Editorial summary grounded in 3 cited references ↓

22841 describes internal spinal fixation that does not attach at each segment. Think spinous process wiring or a single construct that is not tied into multiple anchor points. Segmental pedicle screw and rod systems go to 22840 through 22848 based on the number of segments, not this code.

It is an add-on and is reported with the primary fusion, fracture, or decompression code. Medicare lists it as status B, so payment is bundled and you will not get a separate line paid. Some commercial payers still accept and pay it, which is why practices keep it on claims for those plans.

Coders trip on two things. First, they use 22841 for pedicle screws because the screws are internal. That is wrong. Count the anchored segments and use 22842, 22843, or 22844 for posterior segmental systems. Second, they append modifier 51. Add-on codes are exempt from the multiple procedure reduction, so 51 does not belong on this line.

  • Billed to Medicare expecting payment when the code is bundled
  • Used for segmental pedicle screw constructs
  • No primary spine procedure on the claim
  • Modifier 51 appended to an add-on code

Frequently asked questions

Source · Generated from the editorial pipeline, verified against 3 cited references ↓

01Does Medicare pay CPT 22841?
No. It is a bundled service under Medicare, so the instrumentation is paid as part of the primary procedure.
02What is the difference between 22841 and 22842?
22841 is nonsegmental fixation like spinous process wiring. 22842 is posterior segmental instrumentation across 3 to 6 vertebral segments.
03Can 22841 be billed with pedicle screws?
Not for the screws themselves. Pedicle screw and rod constructs are segmental and use 22842 through 22844.
