What this code covers
Source · Editorial summary grounded in 3 cited references ↓
76881 is a complete ultrasound of a single joint. Complete means the whole joint is examined: the joint space, the muscles and tendons around it, the other soft tissue structures, and any abnormality found. A shoulder study that looks at the rotator cuff, biceps tendon, AC joint, and bursa can qualify. A quick look at the supraspinatus does not. That is 76882.
Sports medicine and ortho practices with in-office ultrasound bill this often, and payers audit it. The report has to list each structure examined, and images have to be stored. A note that says only "ultrasound shows tendinopathy" will not hold up. Bill per joint, with RT or LT.
When the same visit includes an ultrasound-guided injection, the guidance is built into 20604, 20606, and 20611. A diagnostic 76881 can still be billed if the complete diagnostic exam was medically necessary on its own, had its own report, and was not just the pre-scan for the needle. Use modifier 59 or XU and expect scrutiny.

Work RVU vs. total RVU
The work RVU (0.88) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (1.61) adds practice overhead and malpractice, and is what drives the Medicare payment below.
| Work RVU | 0.88 |
| Practice expense RVU | 0.68 |
| Malpractice RVU | 0.05 |
| Total RVU | 1.61 |
| Medicare national rate | $53.78 |

Payment by site of service
Medicare pays different rates by setting. HOPD typically pays substantially more than ASC for the same procedure.
