What this code covers
Source · Editorial summary grounded in 3 cited references ↓
72156 is the standard cervical MRI contrast protocol: images first without gadolinium, then again after it. It replaces billing 72141 and 72142 together.
Spine surgeons order it for suspected tumor, infection, demyelinating disease that mimics myelopathy, and some postoperative questions. For routine radiculopathy or degenerative myelopathy, non-contrast MRI is usually enough, and payers know that. A reviewer will look for a reason contrast adds information.
Common problems: the order says without contrast, the radiologist adds it, and the authorization never gets updated. Or both 72141 and 72142 go out on the same claim. Gadolinium is billed separately in the office using the HCPCS agent code and correct units. When the cervical, thoracic, and lumbar spine are all imaged, each region gets its own code, and Medicare's multiple procedure reduction cuts payment on the additional studies, mostly on the technical side.

Work RVU vs. total RVU
The work RVU (2.23) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (9.52) adds practice overhead and malpractice, and is what drives the Medicare payment below.
| Work RVU | 2.23 |
| Practice expense RVU | 7.13 |
| Malpractice RVU | 0.16 |
| Total RVU | 9.52 |
| Medicare national rate | $317.98 |
| Global period | days |
Payment by site of service
Medicare pays different rates by setting. HOPD typically pays substantially more than ASC for the same procedure.

Common denial reasons
The recurring reasons claims for CPT 72156 get rejected.
Source · Editorial brief grounded in CMS NCCI edits, AAOS coding appeals, and cited references ↓
- 72141 and 72142 billed together
- Authorization for non-contrast MRI only
- Contrast not clinically justified for degenerative disease
- Gadolinium units do not match dose
- Missing professional interpretation
