What this code covers
Source · Editorial summary grounded in 3 cited references ↓
72072 is a three-view thoracic spine x-ray. The third view is often a swimmer's view to see the cervicothoracic junction, or an oblique. Two views is 72070, and four or more is 72074.
Orthopedic and spine practices use it for mid-back pain, suspected compression fractures in osteoporotic patients, trauma, and follow-up of a kyphoplasty or fusion. If you own the x-ray equipment, bill the global code. If you only read films taken elsewhere, bill with modifier 26.
The errors are view counts and overlap. The code must match the views in the report. A full spine scoliosis film is a different code family (72081 to 72084), so do not bill thoracic plus lumbar plus cervical films for one standing scoliosis image. Thoracolumbar junction films have their own codes (72080). When an E/M visit and an x-ray happen on the same day, both are billable; no modifier 25 is needed for the x-ray itself.

Work RVU vs. total RVU
The work RVU (0.22) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (1.16) adds practice overhead and malpractice, and is what drives the Medicare payment below.
| Work RVU | 0.22 |
| Practice expense RVU | 0.92 |
| Malpractice RVU | 0.02 |
| Total RVU | 1.16 |
| Medicare national rate | $38.75 |
| Global period | days |
Payment by site of service
Medicare pays different rates by setting. HOPD typically pays substantially more than ASC for the same procedure.
