What this code covers
Source · Editorial summary grounded in 3 cited references ↓
J7321 covers one dose of Hyalgan, Supartz, Supartz FX, or Visco-3. These are multi-injection hyaluronan products for knee osteoarthritis, given weekly over three to five visits depending on the product. Each visit bills one unit of J7321 plus the injection procedure.
The procedure line is 20610 without ultrasound or 20611 with ultrasound guidance, with RT or LT. For bilateral knees on the same day, Medicare wants 20610 or 20611 once with modifier 50. Some commercial plans want two lines with RT and LT instead, so check the payer. Most payers do not allow an E/M at every series visit; if you bill one, it needs a separately identifiable problem and modifier 25.
Payers want proof the patient failed conservative care first: activity changes, PT or home exercise, oral analgesics or NSAIDs, and usually a steroid injection. Radiographic knee OA should be in the chart. Repeat series generally need at least six months between courses and documented benefit from the last one. Most products are single-dose syringes, so add JZ.
