What this code covers

Source · Editorial summary grounded in 3 cited references ↓

99349 reports a visit to an established patient at their home or residence, including assisted living and group homes, with moderate decision making or at least 40 minutes of total time. Since 2023 the home and domiciliary codes are combined, so the same codes are used for a private home and an assisted living facility.

This fits a worsening chronic condition or a new problem in a known patient, for example increasing hip pain in a resident with a prior arthroplasty, leading to imaging orders and a change in plan.

Homebound status is not required. Match the place of service (12 home, 13 assisted living, 14 group home), and keep nursing facility residents on the 99304 to 99310 codes. If the arthroplasty is still in its 90-day global, a related visit is bundled.

Work RVU vs. total RVU

The work RVU (2.44) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (3.96) adds practice overhead and malpractice, and is what drives the Medicare payment below.
