What this code covers
Source · Editorial summary grounded in 3 cited references ↓
97542 covers wheelchair management: assessing the patient's needs, fitting and adjusting the chair, and training them to propel it and transfer safely. Each unit is 15 minutes.
Ortho practices see it less often than other therapy codes, but it applies after bilateral lower extremity fractures, after foot and ankle surgery with a long non-weight-bearing period, and in older patients after hip fracture who cannot yet walk safely. Training the patient to lock brakes, manage leg rests, and transfer to a toilet or car is billable.
The common issue is overlap with the DME order. The face-to-face and seating evaluation for a power or complex chair has its own documentation path, and the therapy assessment does not replace the physician's order. Billing 97542 for a standard rental chair handed out at discharge, with no skilled assessment or training, gets denied. Document the assessment, adjustments made, training given, and the patient's ability. Use GP or GO for Medicare.

Work RVU vs. total RVU
The work RVU (0.48) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (0.93) adds practice overhead and malpractice, and is what drives the Medicare payment below.
| Work RVU | 0.48 |
| Practice expense RVU | 0.44 |
| Malpractice RVU | 0.01 |
| Total RVU | 0.93 |

Payment by site of service
Medicare pays different rates by setting. HOPD typically pays substantially more than ASC for the same procedure.
