What this code covers
Source · Editorial summary grounded in 3 cited references ↓
97116 is gait training. It covers skilled work on how a patient walks: weight shift, step length, cadence, stairs, and safe use of a walker, crutches, or cane. Each unit is 15 minutes of direct contact.
Ortho uses it constantly after total knee and hip replacement, after lower extremity fractures with weight-bearing restrictions, and after foot and ankle surgery. Teaching a patient to go from toe-touch to partial weight-bearing on crutches is a textbook case.
The documentation has to show skill. Walking the patient down the hall and recording distance is not enough. Describe the deviation (for example antalgic gait, decreased stance time on the operative side), the cues given, the device, and the change. Medicare uses the 8-minute rule and requires GP or GO. CPT instructs not to report 97116 with 97760 (orthotic training) for the same extremity, which matters when a patient is learning to walk in a new brace.

Work RVU vs. total RVU
The work RVU (0.45) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (0.87) adds practice overhead and malpractice, and is what drives the Medicare payment below.
| Work RVU | 0.45 |
| Practice expense RVU | 0.41 |
| Malpractice RVU | 0.01 |
| Total RVU | 0.87 |

Payment by site of service
