Documentation requirements

What must appear in the operative or office note to support the claim.

Source · Editorial brief grounded in 3 cited references ↓

  • Symptoms or exam findings in each of the three limbs, or findings that justified extending the study
  • Muscles sampled per limb with full needle findings
  • Complete study muscle count for each limb
  • Paraspinal levels sampled and which limbs they relate to
  • Confirmation that no same-day nerve conduction study was billed
  • Physician interpretation tying the findings to a diagnosis

What this code covers

Source · Editorial summary grounded in 3 cited references ↓

95863 is a three-limb needle EMG. Think of a patient with bilateral leg symptoms plus one arm, where the question is a diffuse process like polyneuropathy, a motor neuron problem, or multilevel radiculopathy. Ortho and spine groups see it less often than 95860 or 95861. Because it is uncommon, it tends to stand out on review.

The limb-count codes all share one rule. If nerve conduction studies are billed that day, drop 95863 and report 95885 or 95886 for each limb tested. Paraspinals linked to the tested limbs are bundled in.

Medical necessity carries this code. The note needs a reason a third limb was examined, and that reason should show up in the diagnosis codes. A carpal tunnel referral that turns into a three-limb study will usually get cut back on review unless the findings in the first two limbs justified extending the exam, and the report says so.

Work RVU vs. total RVU

The work RVU (1.82) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (6.6) adds practice overhead and malpractice, and is what drives the Medicare payment below.

  • Reported with NCS codes on the same date
  • Diagnosis codes support only one or two limbs
  • Exceeds the contractor's maximum study table for the diagnosis
  • Incomplete muscle sampling in one or more limbs
  • No explanation for extending the study beyond the referral question

Frequently asked questions

Source · Generated from the editorial pipeline, verified against 3 cited references ↓

01When is 95863 appropriate instead of 95861?
When needle EMG of three limbs is medically necessary and no nerve conduction study is done that day. The note should explain why the third limb was tested.
02What do I bill for three limbs of EMG with nerve conduction studies?
Report the NCS code for the total number of studies plus 95885 or 95886 for each of the three limbs.
03Why was 95863 reduced on review?
Usually because the diagnosis or symptoms supported fewer limbs, or the muscle count did not meet a complete study.
