What this code covers

Source · Editorial summary grounded in 3 cited references ↓

85652 is the automated sed rate. Most hospital and reference labs run ESR this way, so it is the code you will see most often for orthopedic infection workups: the painful total knee, post-op wound concerns, vertebral osteomyelitis, and septic arthritis in a child with a limp.

ESR and CRP are the first-line screen for periprosthetic joint infection, and surgeons order them together before deciding on an aspiration. Bill 85652 once per specimen. Do not bill it with 85651 for the same draw. If the result is repeated the same day for a clinical reason, modifier 91 applies.

Some automated ESR systems are CLIA-waived. Offices with an in-house analyzer should confirm its status and add QW on Medicare claims when it is waived. The diagnosis should show the inflammatory or infectious concern. A routine follow-up after an uncomplicated fracture does not support a sed rate.
