What this code covers

Source · Editorial summary grounded in 3 cited references ↓

A sed rate done by hand is 85651, usually a Westergren tube read at one hour. Ortho uses ESR constantly: suspected septic joint, osteomyelitis, periprosthetic joint infection, spine infection, and following inflammatory arthritis. It is almost always paired with CRP.

The code choice depends on the method. Manual ESR is 85651. Automated is 85652. Bill one or the other, never both for the same sample. Manual ESR is a CLIA-waived test, so an office with a certificate of waiver can run it. It is also one of the few waived codes CMS does not require QW on, so leave the modifier off.

Payers expect an inflammatory or infectious concern in the diagnosis: a painful arthroplasty, fever after surgery, a red hot joint, or a known inflammatory arthritis. Serial ESRs during antibiotic treatment are covered when the notes show they are guiding care, but daily repeats without a change in plan get questioned.

Frequently asked questions

Source · Generated from the editorial pipeline, verified against 3 cited references ↓

01What is the difference between 85651 and 85652?
85651 is a manual sed rate. 85652 is automated. Bill the one matching the method used.
02Can I bill ESR and CRP together?
Yes. They are separate tests and are commonly ordered together for infection workups.
03Is a manual ESR CLIA-waived?
Yes. Non-automated ESR is a waived test, and 85651 is on the short CMS list of waived codes that do not need modifier QW.
