What this code covers

Source · Editorial summary grounded in 3 cited references ↓

20999 is the general musculoskeletal unlisted code. Use it only when no Category I code describes the work and no more specific regional unlisted code (shoulder 23929, knee 27599, foot 28899 and so on) fits. Before reaching for it, check Category III codes. CPT requires a Category III code over an unlisted code when one exists.

Medicare has no fee for it. The MAC prices it by hand from your documentation, and so do most commercial payers. That means the claim needs a short cover letter, the operative note, and a comparison code: an existing CPT code with similar time, risk, and work that you are asking the payer to price against. Put the procedure name in the claim narrative field (Item 19 or loop 2400 NTE) so it does not deny for missing description.

The global period is set by the payer (YYY). Expect slower turnaround, frequent records requests, and prior auth for anything elective. Skip modifier 22 on an unlisted code. The payer is already pricing from the work you describe, so put any extra difficulty in the cover letter.

Frequently asked questions

Source · Generated from the editorial pipeline, verified against 3 cited references ↓

01How do I get paid for CPT 20999?
Submit the op note, a cover letter, and a comparison CPT code with similar work. The payer prices the claim manually from those records.
02What is the global period for 20999?
It has no fixed global. Medicare lists it as YYY, so the MAC sets the global when it prices the claim.
03When should I use 20999 instead of a regional unlisted code?
Only when the procedure does not fall within a specific region like shoulder, hip, knee, or foot. Regional unlisted codes are preferred when they fit.
04Can I add modifier 22 to 20999?
No. Unlisted codes are priced on the documented work, so 22 adds nothing and can delay the claim.
