The best Dragon Copilot and Dragon Medical One alternative for orthopedics depends on what you want after you stop speaking. If you want a structured orthopedic note in any EHR on Windows, macOS, or Linux, with no voice training, Mira Dictate is built for that, and Mira Scribe adds ambient documentation with orthopedic coding. If your organization is standardized on Dragon, Dragon Copilot is the natural next step. Suki is an option for multi-specialty groups on its supported EHRs.

What Changed: Dragon Medical One and Dragon Copilot
Microsoft now presents Dragon Copilot as “an AI clinical assistant” and invites existing users to “take your Dragon Medical One experience to the next level,” with a migration section on the product page. Dragon Copilot adds ambient capture, specialty-specific notes, coding suggestions, referral letters, and after-visit summaries.
We did not find a published end-of-sale date for Dragon Medical One. Still, a migration prompt is a good moment to ask what an orthopedic surgeon needs from dictation today.
What Orthopedic Surgeons Need From Dictation
- Orthopedic vocabulary without training. Implants, eponyms, approaches, and anatomy.
- Structured output. A note organized into sections, not a transcript you rearrange.
- Clean capture. Side conversations in clinic or the OR left out of the note.
- Direct EHR entry. Text lands in the active field, including ModMed.
- Every desktop platform. Windows, macOS, and Linux.
- Laterality you can trust. It drives modifiers -LT, -RT, and -50.

1. Mira Dictate: Orthopedic Dictation With Structured Output
Mira Dictate is a desktop app for Windows, macOS, and Linux built on hundreds of thousands of orthopedic encounters.
- Captures intent, not just audio. It writes down what you want documented and leaves out side conversations and staff interruptions.
- Ortho vocabulary from day one. Implant systems, surgical approaches, eponyms, and subspecialty terms with no user training.
- Structured clinical output. “MRI left knee” becomes a structured order, and op note content arrives organized for review.
- Direct insertion. Press a global hotkey and text goes into the active field in any app: no clipboard, no paste.
- Personal dictionary and phone mic. Add your own terms, and use your phone as the microphone when you are away from your desk mic.
- Template commands. Say “next field” to move through a template, or insert a saved template by voice.
- HIPAA. Encrypted processing, nothing stored on the device, BAA included.

2. Mira Scribe: Ambient Documentation and Orthopedic Coding
If part of your reason to look at Dragon Copilot is ambient documentation, Mira Scribe covers that for orthopedics. It drafts clinic notes from the visit, captures laterality on 100% of operative notes, and codes with CPT bundling, modifier assignment, E/M leveling, and ICD-10 mapping at 97% first-pass accuracy. It integrates natively with Epic, athenahealth, ModMed, eClinicalWorks, NextGen, and DrChrono, and builds new EHR integrations in one to four weeks. Scribe also drafts orders, referral and work-status letters, and follow-up instructions, and has a dictation mode for op notes.
3. Dragon Copilot
Dragon Copilot is the natural path for organizations already standardized on Dragon, especially those that use it across physicians, nurses, and radiology. Microsoft lists web, mobile, desktop, and embedded EHR options for physicians.
4. Suki
Suki offers ambient documentation and a dictation product with custom voice commands, auto-texts, and support for Nuance PowerMic and Philips microphones. It lists Epic, Oracle Health, athenahealth, and MEDITECH integrations. See Mira Scribe vs Suki.
When to Stay With Dragon
Stay if your surgeons rely on Dragon voice commands, shared AutoText, and PowerMic Mobile, and your notes already need little cleanup after dictating. Those are real strengths, and switching has a cost.
Look at alternatives if dictation is fast but organizing the note still takes time, if you need Mac support, or if your Dragon renewal now requires a broader Dragon Copilot decision.
How to Run a Fair Test
Use the same three dictations in every product:
| Dictation | What to check |
|---|---|
| Knee arthroscopy op note (CPT 29881) | Side, findings, structure, implants |
| Clinic follow-up for knee OA (M17.11) | Exam and plan in the right sections |
| Letter to a referring physician | Formatting and time to finish |
Count the edits before each note is ready to sign. Use our free CPT, ICD-10, and modifier references to check coding details.
